Hi,
It is given that • wasted material • wasted labor • credits, adjustments or refunds to customers • lost business due to poor quality or lack of capacity are the factors to be considered, others?

However is there anyone holding up a kind of template or form that could easily be filled in? Am searching the net for 2 hrs now and can't find any.

Really appreciate your favor; just in case kindly send to [email protected]

A million thanks!

hi george,

this is a very good question. what i think is the matter/topic involved is too broad. maybe we should look into items separately like the cost of customer returns (cost model). with regard to the cost of "waste", the TPM or (Lean) Six Sigma guys could probably answer this. we could also ask how a customer charges the supplier for shipment delays. Quality and Cycle Time (or throughput time) are 2 distinct areas too. although these two are interlaced in the concept of Six Sigma.

Regards.
